We are making updates to the client chart to help you capture more information about what happened during your client visits! Here are the highlights of what’s changing:
1. Keep track of who you reached and how you reached them
Now when you create notes you’ll be able to choose who you reached – the client, their family or friend, a healthcare provider – and how you reached them. For encounters where you met in person, you’ll have more options for capturing where those encounters happened.

In addition, when you don’t reach anyone, you can still keep track of who you were trying to reach.

2. Record documentation time
Keeping thorough and accurate client records takes time and care. Now you can record how much time it takes with the “Documentation Time” field. You’ll find this field right under where you type notes.
3. Edit finalized notes
Hit “Finalize” for a note before you meant to, or realized after you finalized that you need to make a change? Now you can edit your own notes even after they’ve been finalized. If you’re a manager or director you’ll be able to edit notes of all users across your organization.
4. See the “Date of Encounter” for historical notes
Currently in the Notes History section, the dates you see are the dates the notes were created. We’re updating these dates to display the “Date of Encounter” for the note. This will make it easier to find notes based on when you met with your client.
5. For managers and directors: Leave more detailed Manager Review notes
The Manager Review function has been updated so it’s easier to leave a brief note about your discussion with a CHW about a client or your review of the client chart. Access the Manager Review note through the “Manager Review” button or by creating a “Manager Review” note directly in the client chart.

6. “Enrollment” note renamed “Outreach” and addition of SDOH Z code explanations
There are 2 more changes in note creation that you’ll see:
- We’ve renamed the “Enrollment” note type “Outreach” to make it clearer that it’s the note type for all outreach attempts, regardless of whether the client enrolls, declines, or is not reached.
- When you select an SDOH Z code for a client visit, you’ll see a new field called “Explain SDOH Z code”. When you click on this you’ll see a short reason for why a Z code is relevant for a client. Select the reason to confirm your Z code selection.
Why
Having more detailed note information helps everyone – CHWs, supervisors, and program sponsors – have a fuller picture of the work being done. This information will help you spot trends in how clients are being supported and identify strengths and opportunities in the work being done. In addition, if your organization is planning to bill for CHW work, these updates will help you capture the data needed to submit claims.
