A new feature called Care Gaps came to the IMPaCT app in April!
What it is: There’s a new section in the Client Profile dedicated to care gaps. A care gap occurs when there is a difference, or "gap," between the medical care a client needs and the care they have actually received. With this update, you’ll be able to see which of your clients have common care gaps and the last date when the care gap was known to be present. In the example below this client has a “Breast cancer screening” care gap that was current as of 03/17/26.
The care gaps that are currently supported are: breast cancer screening, colon cancer screening, lowering HbA1c, blood pressure management, dental care, child and adolescent well-care, follow-up after mental health hospitalization, follow-up after hospitalization, prenatal visit, and post-partum visit.
Why it’s important: Care gaps are important opportunities to support clients. CHWs can really shine in helping clients understand why the recommended care is important and in removing barriers to getting care. In addition, addressing care gaps is often an important quality measure for program leaders who sponsor and champion CHW programs.
How to use it:
- To start seeing care gaps, ask your program leader to work with your IMPaCT account lead to send care gap information to the IMPaCT app.
- Once this information connection is established you’ll start seeing care gap information in your client charts!
