A new feature called the Health Tracker is arriving in the IMPaCT app on February 13, 2025!
What: With the Health Tracker you can record your client’s health metrics like weight, daily steps or smoking. You can even create your own metric based on what your client is working on: trips to the senior center, visits with a granddaughter or walks around the block!
Why: The Health Tracker helps you track your clients’ journey toward better health. It will help you celebrate wins along the way or course correct if things move in the wrong direction. Program leaders can use the Health Tracker to get concrete data on the outcomes that CHWs and clients achieve together.
How: All these updates will be available when you sign in on February 13!

The basics:
- Go into a client’s chart. You’ll see a “Health Tracker” heading underneath the screener – click on the “+” to get started!
- Enter a value for one of the categories on the list – hit Confirm and see your value show up in the Health Tracker on its own tab.
- If you’d like, pick “My own metric” from the categories on the list and follow the steps to create your own metric.

Pro-tips:
- We know that most of your clients have a lot of different health conditions. It can be overwhelming to work on all of them at once. In the IMPaCT model, we recommend picking a primary metric: the one that you and your client are laser focused on. Try selecting a primary metric in the app: you can do this by selecting the star next to the name of the category on its tab.
- Enter a value by mistake? Try deleting or editing a value that you entered.
