Let's learn how to navigate the client list.
After you log in as a CHW you'll be taken to the Clients where you'll see a list of clients in your organization. This list includes clients at all different points in their journey -- from those who have yet to enroll to those who have graduated and completed the program. There's a lot of things the client list can do and in this article we'll get you oriented to the basics in the default view.
Filters
When you look at your client list you'll see filters and a search bar across the top.
1. Status: A status tells you where a client is within the program. Depending on your program, you may see clients in statuses such as pre-outreach, enrolled, and pending - we'll get into those in the status management article.
2. Program: This filter is first from the left is where you'll select the program you'd like to look at the client list for.
3. CHW: This is how you can sort clients by their assigned CHW within your program. For example, if you selected your name, you'd see all of the clients assigned to you.
4: Search Bar: You can use the search bar to search a client by name, MRN/Client ID or IMPaCT ID. To the right of the search bar, the circular button with the "up" arrow allows you to upload a Client list if you have a large list of clients through your organization. We'll cover this in more detail in the Uploading Client Lists article.
Let's say this is your client list and you'd like to filter your list by clients awaiting outreach to learn more about your program. By selecting the drop-down Status menu and selecting "Pre-outreach", the list will automatically filter to only the clients whose status is "Pre-outreach". Clients in any other status will not be displayed.
Columns and sorting
Columns
Beneath the filters you'll see a list of clients with standard information for each client arranged in columns. Here's what each of the columns mean:
Star: Use this to mark a client of interest that you might want to cover in a run-the-list meeting or keep in mind for any other reason. Both CHWs and supervisors can star and un-star clients.
MRN/Client ID: This is the client’s MRN or client ID. If you hover over this, you will see the client's IMPaCT ID number which is specific to the IMPaCT application. The IMPaCT ID is the identification number you will use when communicating about a client so that you do not share PHI. For example, if you write to our helpdesk, please use the IMPaCT ID.
Name: The client's name -- first name and last name. When you hover over the client’s name you will see the Client’s IMPaCT ID, their age, phone number, and preferred speaking language.
Date eligible: The most recent date a client was eligible for their program. For example, if Wade Warren came in on an eligibility list on 6/1/23, then again on another eligibility list on 6/12/23, then this date gets updated to 6/12/23.A few other nuances to know about this column is that:
- This date is also updated when a client’s program is changed to reflect their new eligibility date for the new program
- Or, if a client goes back to an outreach status (pre-outreach, pending, maybe, not reached) from a close-out status (graduated, withdrawn, lost to follow-up), then this date is also updated because they have a new eligibility.
- Once a client is enrolled this date stays the same for the duration of their enrollment

If a client is Not Reached, Declined, Graduated, Lost to follow-up, Withdrawn, Ineligible, or Terminated for coverage loss, and they are re-eligible an icon will appear next to the date eligible. If you hover over the icon, you will see the most recent date the client became eligible and the program they are now eligible for.
Ref. : Whether the client has been referred to your organization's CHW program, for example by a primary care provider, a care coordinator, etc.
Status: The current status that the client is in within the program. You can also click on the status to update your status and leave a note!
Program: The specific program that the client is associated with in your organization. (Example: General Adult Program)
CHW: This is the CHW assigned to the client. This can be blank if the client has not yet been assigned a CHW. This is what you will click to navigate to the client’s chart.
Hosp. : This column shows you the date of a client’s most recent hospitalization. If available, when you hover over the date, you will see the location where your client was hospitalized.
Op: This column shows the date of the next upcoming or most recent outpatient visit for your client.
Sorting
Each column is sortable by clicking the up and down arrows. To undo your sort, just click again on the arrow.
