The client profile holds all of the important details about clients you're working with. How about a deep dive look at the client profile?
Navigating to the client profile
To reach the client profile, click the client's name on the Client list. This will open a pop up window that takes you to the "Our Work" tab. In the Our Work tab, you'll see "Profile", which highlighted in the orange box below.

Client demographic and contact information
The rest of the client profile screen is dedicated to demographic information. It has fields for standard demographics such as name, date of birth, the client's MRN or ID within your organization, phone number, etc. You'll see fields for pronouns and preferred name which are important for affirming your client and making them comfortable. All fields marked with an asterisk* are mandatory; the rest are optional, but helpful to have.
The referral section indicates whether the client was referred to the program or if they were identified as eligible based on set criteria for your organization. It's nice to know this because you may want to get in touch with the referring person to let them know how your work with your client is going.
The Insurance tab allows users to enter Primary and optionally Secondary insurance details, including the insurance category, plan, member ID, and group number. If the insured person differs from the client, additional fields for the insured's personal information are available. There is an Insurance Notes text field at the bottom to capture any additional details.
The Care Gaps section displays care gap indicators for a client and is populated through data feeds. Each care gap appears as a row showing the gap category and date/time. Even if the indicator becomes inactive, it will remain visible so that there is a record of your client’s care gap history.
The different sections of the client profile can be collapsed and expanded to help you quickly find the information that you need. If any required information is missing or if any information entered is invalid (for example, a phone number is missing a digit), an orange alert will appear at the section header. Take a look within the section to see what is causing the error; once it's resolved the orange alert will disappear and the Save button can be clicked.
Of note, when you're creating a new client the app will check whether the client already exists in the system and, if they do, prevent you from creating a duplicate client. Learn more about how duplicate prevention works here.
Client profile information can be updated at any time. Don't forget to hit the Save at the bottom right when updating!
