Outreach to Enrollment
After you have outreached a client and they have agreed to enroll to receive CHW services there are a couple of things you must do.
First, update your client's status to Enrolled. You can access the status modal both from the client list or the client’s chart by clicking on the Status button.
Then, in the Status modal you must collect your client’s consent by clicking on the checkbox that appears. You must click the Confirm button to save your status change and client’s consent to CHW services.
Enrollment to MPI
Once you have enrolled your client you will need to complete the MPI workflow. This flow consists of:
- Meet the Person Interview Note- Note used to document the client’s shared life story, resource/support needs, and the roadmaps you’ll work on together.
- Screener- A screener to help you collect your client’s needs. For more information about the Screener visit this helpdesk article.
- Snapshot- A quick summary of your client’s life story and goals.
To help you track where you are in the MPI workflow there is an MPI indicator with each of the elements of the workflow found under the Status button in the client’s chart. As soon as you update your client's status to Enrolled, this indicator will appear.
When you are ready to complete your Meet the Person (MPI) note, you will also have the option to submit your Screener and Snapshot. This way, you can complete the MPI workflow all in one go. Each button opens up their own modal that can be closed without losing your note content. If you choose to complete some of your MPI elements later, you can use the indicator to track what remains to be completed.
Pro-tip: If a Screener was filled out for a previous enrollment you must submit a new Screener for it to count towards your client’s new enrollment and your MPI indicator.
When you click on the Snapshot button, the Snapshot modal will appear. You can also close out the Snapshot modal and type directly into the Snapshot area if you prefer. As you type your Snapshot, it will autosave and the indicator will update the Snapshot as done so make sure that the content of your Snapshot is what you want before closing out. Of course, you can always edit the Snapshot if you need to by clicking on the pencil icon.
Once you have completed all three elements of the MPI workflow the MPI indicator will have each of the elements checked off. In the client list, you will also see ‘Yes’ under the MPI column.
Pro-tip: If the MPI column displays as “ No”, you can now click on the client’s name and visit their chart and use the indicator to learn more about which element is missing. Remember, each time you enroll a client you must complete a new Screener and MPI Note. The only element of the MPI workflow that carries over from previous enrollments is the Snapshot.
