Introduction to the Health Tracker
The Health Tracker is a tool you can record your client’s health metrics like weight, daily steps or smoking. You can even create your own metric based on what your client is working on: trips to the senior center, visits with a granddaughter or walks around the block!
The Health Tracker is a great companion tool to your Roadmaps that helps you track your clients’ journey toward better health. In this article we'll go over how to get started with the health tracker and common actions you'll take when using the Health Tracker.
Orientation to the Health Tracker
You can find the Health Tracker under the Screener section in a client chart. You'll see that there's a plus button there -- clicking on that button opens up the Health Tracker.
Now you’re looking at the Health Tracker! Below is what the Health Tracker looks like before you’ve entered any information. Let’s get you oriented to its different components.
- Category: this is the type of metric that you want to track. The list includes common health metrics like blood pressure, smoking, and exercise. If you want to track something that you don’t see on this list you can create your own category.
- Date, Time: This is the when the metric was taken. For example, if your client had their blood pressure taken in clinic 5 days ago, you’d put in the date from 5 days ago. You can add time if you like, but it’s not required.
- Value: This is what the metric was! For each of the different categories you’ll see the units are already there for you. For blood pressure, enter only the “top” number. The top number is the one that tells you how your blood pressure is doing.
- Source: Here you’ll want to enter where the data came from. Sometimes it will be from the client’s medical record, or they could tell you (“Self-reported”), or you could measure it together ("In-person measurement).
Adding metrics to the Health Tracker
To enter a metric, go ahead and choose the category of the metric, then go through and add in values for each of the fields.
After you add a metric, you'll see the category gets its own tab in the health tracker where you can see the value you just entered and, like other lists in the app, you’ll be able to sort based on the date, value, or source
Primary metric
Once you’ve been working with your client a little while you could be tracking a few different metrics together. As part of the IMPaCT model, we recommend picking one metric to laser focus on as this increases your client’s chance of success. This metric is called the primary metric and it’s easy to assign this in the app – just select the star next to the name of the metric. When you select a primary metric, the metric will appear in the first tab in the Health Tracker. When you un-star it, it goes back to its position in alphabetical order
Editing or deleting metrics

Now let’s say you entered a metric and accidentally type in the wrong value. Not to worry! You can go back and edit values that you’ve already entered. To do this, click on the value that you want to change and make your update. Then hit Enter when you’re done.
Deleting
Now let’s say you entered a value you didn’t mean to. For example, you accidentally put a value for blood pressure in weight. No problem! When you hover over the line you want to delete, you’ll see a trash can. Go ahead and select it to delete the entire entry.
Creating your own metric
There is one more feature of the Health Tracker we want to share and that’s the ability to create your own metric with your client. That way, no matter what’s important to your client, you can help them stay on track. To create your own metric, go to the “Category” list and select “My own metric”. Once you do that you’ll see another line appear beneath this.
- Go ahead and type in what your client's working on -- for example visiting their daughter more regularly.
- Then, select whether you want it to go up or down. In this case, you want this to go up, so you would select "Increase"
Then you can go up to the top and enter the date, value, and hit Confirm. Then you'll see the category you entered appear on its own tab in the Health Tracker.
